I am struggling to attack the third ball and in fear of the shot missing I end up playing a push rather a flick or a slow spiny topspin a video answer would be very useful

Playing the third ball is something that is with persevering with. It is an important part of advancing your game.
If you are worrying about missing then use it more in a training situation initially. Remember that the more topspin you put on the ball the safer the shot becomes. In training, play a game where your partner has to push the ball long to you and you have to topspin that ball... then after that it is a free rally. When playing penhold players it is important to remember to first analyse your opponent as you would if they used any grip. What are their strengths and weaknesses? A common weakness of penhold players is their forehand block especially against a slow spinny topspin. Their backhand may be a weakness if they use the traditional style, or you may wish to explore the crossover point if they are using the reverse penhold backhand.
