How tight do you squeeze the forefinger on to the paddle? I have been using a relaxed hold on to paddle with forefinger and hand.

Keep the grip as relaxed as possible. This will allow you more flexibility with your wrist and also better feel.
When you need to reach that really wide ball the regular shuffle step just won't get you there in time so you need to use the crossover step. This is essentially a running action to get you to the ball as fast as possible.
